A few things to note before you fill out this form:

Before you fill out this form, please double-check that you can commit to attending the performances you’re about to request. Check for Pro-D days, school events, and other conflicts.
Tickets are in high demand. Once you have submitted your request, we will reach out to confirm whether we are able to provide you with tickets.
We prefer to accept payment by Electronic Funds Transfer or credit card. Please consult with your school’s accountant or payments officer. If neither option is available, we will accept payment by cheque. Please note that payments by cheque have earlier deadlines than other forms of payment. Dates are listed later in this form.
